A Questionnaire Commission is for the weeks before anyone is handed a form. We write the questions for a customer group you can describe, set the answer choices so they can be counted later, and leave you a field note your own staff can follow.

What you leave with

A questionnaire in Thai, English, or both, plus a field note on who should receive it, when, and what to do with a refusal.

Drafted in East Samwa from a briefing you give in person or by letter; no field visit is included.

Inside the fee

  • A briefing on the decision the future reading must serve
  • Up to twenty closed questions and four open prompts
  • Answer choices written so a later count will not collapse into a single average by accident
  • Wording in Thai, English, or a paired version
  • One revision after you have tried the draft on three people from the audience
  • A one-page field note for the person handing the form out

  • Printing, translation beyond Thai and English, or a reading of the returns
  • A sample-size calculation dressed up as certainty
  • Questions about clinical condition, credit, or anything we would not want written on a card in a lobby

How the days run

  1. Brief. You tell us who will be offered the form, what you might change if the answers lean one way, and what you already ask on a comment card or at the desk.
  2. Draft. We send a draft with a note beside any question that will be hard to read later. You try it with three people who resemble the audience.
  3. Field note. After the revision we send the form you can print and a short note your staff can keep at the point where the form is offered.

What to send before we begin

  • Any form you use now, even if you dislike it
  • A description of the moment the customer would be asked
  • The decision you expect to make with the answers

Limits we will name at intake

  • We will not load a form with every curiosity in the building. Twenty closed questions is the ceiling on this fee.
  • If the try-out with three people shows the audience cannot finish the form in the time you have, we cut it. That cut is the revision, not a new commission.

The fee, in plain terms

28,000 THB for one questionnaire and one revision, in one or both of Thai and English. A paired form that is truly two different instruments, not a translation, is quoted separately. The fee does not include a later Survey Insight Briefing; that is a second commission once the cards are back.
